Medical outreach, in particular, continues to grow across the country with health centers increasing these services and other independent organizations emerging regularly. To best address complex and systemic inequities, commitment to equity is cross-departmental within the City of Grand Rapids including the GRPD, GRFD, and Community Development Department.Fundamental to the HCH model of care, outreach is an umbrella term for serving people outside of the four walls of the clinic, which may include providing medical services on the streets (“street medicine”), using a mobile medical van, or deploying outreach specialists to build relationships with those who cannot or will not access services in the physical clinic. Racial inequity exists among persons experiencing homelessness. Additionally, the HOT team will be obtaining a variety of trainings to best serve the community in respect to vulnerable homeless populations. The GRPD strategic plan proposes several ways to execute on this commitment including creating equity centered training and staff development for community engagement and using data regarding community sentiment gathered through community surveys to constantly evaluate and improve police services while ensuring equity.
